All Official High School Transcript Requests require the following information:
- Student's full name (at time of graduation) and current name
- Graduation year
- Date of birth
- Full name and address of institution requesting transcript
- Daytime telephone number of person requesting the transcript
Requests may be submitted by the following methods:
- Send email to: firstname.lastname@example.org (preferred)
- Fax to 440-998-6143
- In person Monday-Friday from 7:00 a.m. to 3:30 p.m. (during the school year).
Complete the Transcript Request Form below and fax or drop off to the Guidance Office
Please allow 2-3 days processing for ALL transcript requests.
